How to obtain a copy of
a Death Certificate
Application for Vital Records Form Adobe Acrobat Form
To obtain
a death certificate, the interested party must indicate the reason for
applying, such as: property right interest, personal interest, legal
representatives, etc.
For
the purposes of genealogy, the death certificate must have been on file for
at least 20
years.
The fee is
$10.00
for each death certificate and $6.00 for each additional copy of the same
certificate.
To obtain a
death certificate, complete the adobe acrobat
Application for Vital Records Form, include a money
order made payable to County Clerk and mail to:

Note: All vital
records (birth certificate, death certificate and marriage license) are not
public records, and therefore are not subject to the Freedom of Information
Act.
